Print or type your correct name in the "New Registered Owner" section on the reverse side of your title. Also fill out Form REG 256, Miscellaneous Statements of Fact under part H "Name Statement" and sign the bottom. Submit both of these items to the DMV. (You can also submit these documents by mail).

Give the lien holder the completed Name Statement (Form REG 256). They will have to submit this along with the Certificate of Title to the DMV to have your name changed.

The title must be presented to a county MVD office. The owner(s) must sign off on the "seller" portion of the title, and a new assignment is made on the "convey to" portion of the title showing the new name or names. Notarization may be required depending upon the title.
